Presentation of a child with kidney disease

Kidney disease in children often remains undiagnosed due to non specific signs and symptoms. The tell tale signs of kidney disease in children are:

  • Early morning puffy eyes.
  • Blood in urine.
  • Frothy urine
  • Increase or decreased urinary output
  • Growth retardation
  • Unexplained fever
  • Anemia refractory to Iron, vitamin B12 and folic acid
  • Bony deformities
  • Frequency, urgency and painful micturition
  • High Blood Pressure

Congenital & hereditary abnormalities of kidney and urinary tract

With the advent of antenatal ultrasonography, congenital abnormalities of the kidney and urinary tract are the commonest anomalies reported in fetuses. Today the process of early detection and management of any abnormality through ultrasound is also called as Fetal Medicine. The common congenital & hereditary abnormalities of kidney and urinary tract include:

  • Cystic And Dysplastic Kidneys

It is essential that these defects are detected as early as 16- 20 week of gestation. Later they are incidentally detected by sonography of abdomen while evaluating a child with pain abdomen, UTI and other diseases. Usually children with one functioning kidney lead full healthy lives. However some children may develop acute and chronic kidney disease in their lifetime.

Hydronephrosis is due to impaired drainage of urine, leading to overfilling of urine in the kidney and its subsequent swelling. Hydronephrosis is found in 1 in 100 babies. In 40-80% of babies the problem is transient and resolves soon after birth or within few months. However in rest it is caused by slow and obstructive drainage (Posterior urethral valve, Pelvi-ureteral junction obstruction) or due to back leak of urine (vesico ureteral reflux). Intervention is required in 5-15% of patients but chances increase as severity increases.

Polycystic Kidney Disease

Polycystic kidney disease is characterized by large sized kidneys due to fluid-filled cysts—abnormal sacs—that increase the size of kidneys over time. These cysts occupy the normal functioning kidney tissue. It may be autosomal dominant in whom one parent is affected or autosomal recessive in whom both parents carry the mutated gene. Gradually the kidney functions decline and leads to end stage renal disease in 3rd or 4th decade of life in autosomal dominant. In autosomal recessive variety there is early onset of renal failure in first decade of life. Usually it remains asymptomatic but many children have recurrent urinary tract infections, hypertension, kidney stone, etc which hasten deterioration of renal function.

Alport Syndrome

Alport Syndrome is an inherited disease caused by gene mutation in a type of protein called collagen which is a structural component in various tissues like the kidney, inner ear and eye. It affects males more than females.

Children with Alport Syndrome present with hematuria (presence of blood in the urine) which is usually microscopic sometimes after cold/flu. Gradually proteinuria ensues which may damage glomeruli by fibrosis and lead to scarring in kidney and progressive decline in kidney function.
