The process of fertilization of an egg by a sperm which takes place outside the body is described as ‘In Vitro Fertilization’. Why is it done?

Risks – The risks associated with In Vitro Fertilization include –

Multiple Births – This occurs in cases where implantation of more than one embryo takes place in the uterus. Multiple births come with complications such as neonatal morbidity, early labor, low birth weights, and so on.

Ectopic pregnancy – The assisted reproduction technology can result into an ectopic pregnancy wherein the embryo gets implanted outside the uterus, usually the fallopian tubes.

Birth defects – Certain cases where infants are born due to IVF had defects like anorectal atresia, septal heart defects, cleft lip etc. were reported. Parental factors like age and ill-habits could be the cause behind the defects.

Other risks could be miscarriage, stress, ovarian cancer, and ovarian hyper stimulation stimulation syndrome.

Intrauterine insemination (IUI) is a procedure for artificial insemination that can be used for treating both male and female infertility. It is the most common type of artificial insemination. In earlier time, artificial insemination procedure was done by placing the sperm in the vagina, but it was never as successful as the current one is.

In intrauterine insemination, the man’s sperm is directly placed into the woman’s uterus to facilitate fertilization. This helps the sperm to reach the uterus, preventing any complications in its journey. The goal of IUI is to increase the chances of fertilization by making a large number of sperms reach the fallopian tubes. IUI gives the sperm a head start, after which the sperm has to travel and fertilize the egg on its own.

Why is IUI done?

Intrauterine insemination may be a good option for artificial insemination when the man has low sperm count or when the woman has endometriosis. Woman’s older age, low sperm count or poor egg quality, or severe fallopian tube damage can be the reasons behind problems in getting pregnant.

How is it done?

IUI is timed to overlap with ovulation (release of the eggs) period of the woman. The woman is given medications to stimulate development of multiple eggs. Sperm are concentrated from a semen solution, washed and are placed directly in the woman’s uterus.

In most cases, there are no obvious signs of infertility. Intercourse, erections and ejaculation will usually happen without difficulty. The quantity and appearance of the ejaculated semen generally appears normal to the naked eye. Medical tests are needed to find out if a man is infertile.

There are few medicines that are recommended for the treatment of infertility. Clomiphene is given as it stimulates your ovaries to release eggs. Metformin is used to treat polycystic ovary syndrome. New drugs containing myo-Inositol and acetyl cysteine have been found beneficial in correcting the underlying physiology of PCOD. However self medication is a not recommended. Artificial Insemination is a procedure used to treat infertility that involves direct insertion of semen into a woman’s womb. Commonly used method of artificial insemination includes IUI (Intrauterine Insemination) and it has the best success rate. Other types of artificial insemination include:

  • IUTPI (Intrauterine tuboperitoneal insemination)
  • ICI (Intracervical insemination)
  • ITI (Intratubal insemination)

Both women and men can have problems that cause infertility. About 30% of infertility cases are caused by women’s problems. Another 30% of infertility problems are due to problem in male partner. The other cases are caused by a mixture of male and female problems or by unknown problems.

Causes of infertility in female:

  • Ovulatory Dysfunction: It caused by PCOD, decreased ovarian reserve, premature ovarian failure. Ovulatory dysfunction contributes to 30 to 40 % of female infertility.
  • Tubal Factor: Tubal factors accounts for 30 to 40% of infertility. Tubal factors include damage of fallopian tubes because of previous PID, previous pelvic or tubal surgery or endometriosis. Other causes of tubal damage include isthmica nodosa or benign polyps within tubal lumen.
  • Unexplained Infertility: Accounts for10 to 15% cases. In this psychological and gynecological support is to be provided.

Infertility refers to an inability to conceive after having regular unprotected sex for a period of one year. Approximately 85 to 90 % of young and healthy couples conceive with in 1 year. Infertility therefore affects 10 to 15 % of couples. This is a reason for concern for women who have being to conceive.
