A one-year-old child was brought to the Neurosurgery OPD at Paras Health with a progressively enlarging head and delayed developmental milestones. Following a detailed clinical evaluation, an MRI of the brain revealed aqueductal stenosis with obstructive hydrocephalus, a condition in which the normal flow of cerebrospinal fluid (CSF) is blocked, leading to excessive fluid accumulation inside the brain.

The child's family had sought multiple medical opinions and had been advised to undergo a Ventriculo-Peritoneal (VP) Shunt procedure, the conventional treatment for hydrocephalus. However, after careful assessment, the neurosurgical team decided that the child was an ideal candidate for Endoscopic Third Ventriculostomy (ETV), an advanced, minimally invasive procedure that restores the brain's natural CSF circulation without implanting a permanent shunt.

During the surgery, a high-definition neuroendoscope was carefully introduced into the brain's ventricular system. A tiny opening was then created in the floor of the third ventricle, allowing the trapped cerebrospinal fluid to bypass the blockage and flow through its normal pathways around the brain.

Compared to traditional VP shunt surgery, ETV offers several long-term advantages. Since no synthetic tubes or valves are implanted, patients avoid lifelong dependence on shunt hardware and the associated risks of shunt blockage, mechanical failure, and repeated revision surgeries. By re-establishing the body's natural CSF drainage, ETV also reduces the long-term risk of infections and provides a more physiological solution for selected patients with obstructive hydrocephalus.

The successful treatment of such a young child highlights not only the expertise of the neurosurgical team but also the importance of a multidisciplinary approach. Safe execution of ETV in infants requires seamless coordination between an experienced neurosurgeon, a pediatric anesthetist, and a pediatrician, supported by a tertiary care hospital with a fully equipped Pediatric Intensive Care Unit (PICU).

The child recovered well after surgery and is being closely followed for neurological development and continued improvement.
